The International Secretariat has developed two beneficial ownership model declaration forms to support the implementation of Requirement 2.5 of the EITI Standard, which implementing countries can consider using. These are:

The model declaration form for basic data collection, available in Excel and Word format.
- The model declaration form for high quality data collection, compatible with the Beneficial Ownership Data Standard (BODS). The file is accompanied by a guide for form designers and administrators. The file is in Excel format but can be adapted as an online form.

Implementing countries may consider using the form for high quality data collection if they:
- Aim to collect information on legal and beneficial owners using an online form;

Are building their data collection using existing data available to the government (e.g. from a commercial registry or cadastre);
- Wish to share or link the collected information with other government data;

Plan to publish most of the collected data publicly, through a register and/or an API.
